Record a failed initial load or retry of path: first unwind the ladder pass
the load started (a no-op for a loader without one), then record the failure.
A progressive loader advances its cursor as each rung arrives, before the
concat and commit that can still throw; left advanced, the retry would resume
past the failed prefix and attempt a larger allocation (#2426, see
loaders/progressive/pass-rollback). The update sweep and the refinement
wrapper unwind for the same reason.
